- Prescription Assistance Program - The Prescription Assistance Program assists eligible patients in obtaining medications at no cost. The program utilizes The Pharmacy Connection of the Virginia Health Care Foundation. Eligibility includes those patients that do not have prescription insurance coverage and meet income guidelines. - Medicare Part D counseling and enrollments. Representatives at Bath Community Hospital can help you understand and enroll with Medicare Part D (drug coverage). Our representatives are available Monday through Friday between the hours of 8:30am and 5:00pm. - Advance Medical Directives - In 1990, Congress passed the Patient Self-Determination Act. It requires health care institutions to tell patients and the people in their communities about their rights under Virginia law to make decisions about their medical care. These rights include the right to accept or refuse care and the right to make advance directives about their care. Advance Medical Directives include Living Will, Medical Power of Attorney, and Organ Donation. For assistance contact Lorri Batten, Social Worker, at 540-839-7188.
