YOUR HEALTH RECORD
Every time you register at the hospital a record
of your visit is made. This record usually includes your
symptoms, results
of tests and exams, as well as your diagnosis, treatment
and plan for future
care.
Although your health record is the physical property of Bath
Community Hospital, the information belongs to you. You have
rights to your health record that are guaranteed and defined
by law, including the rights to inspect and obtain a copy
of your health record, change your health record, obtain an
accounting of disclosures of your health record and request
restrictions on certain uses and disclosures of your information.
Our hospital is required to maintain the privacy of your health information,
accommodate reasonable requests you may have regarding your health
information, and notify you if we are unable to agree to a requested
restriction.
